Select Address Book Fields
You can format what needs to be printed on the labels. First check the required address book fields. In the image below, Title, First Name, LastName, ... are checked and they will appear on the label (provided that the specific contact has these fields). Each field has some properties. In the image below, the field "Company" begins in a new line. If you wanted it to be appended to last name, then you would uncheck this property.

Additional options
Font
The font type and size that you select will apply to all fields.
Adjust Margins
In some cases you have the correct label size but labels are printed slightly out of alignment. You can use this option to move them slightly up or down left or right.
